This Vida Health Personnel Privacy Notice (“Personnel Privacy Notice”) describes how Vida Health, Inc., in conjunction with Vida Medical, PC, a California professional corporation, Vida Medical, P.A., a Kansas professional association, Vida Medical, New Jersey, P.C., a New Jersey professional corporation, and Vida Medical P.A., a Delaware professional corporation, (collectively, “Vida”, “we”, “us”, or “our”) collect, use, and disclose the Personal Information (as defined below) we collect about you in the context of your role as a Vida employee, contractor, job applicant, or prospective candidate. If you interact with our website, use our services, or otherwise engage with us outside of the employment context, the privacy policy provided in connection with that interaction applies instead, not this Personnel Privacy Notice.

This Personnel Privacy Notice does not form part of any contract of employment or other contract to provide services and does not give you any contractual rights.

Whenever you interact with us on behalf of another individual or entity, such as if you provide information about a dependent, you must obtain their consent (or have the legal authority without consent) to share their Personal Information with us.

We may update this Personnel Privacy Notice from time to time. Any updated Personnel Privacy Notice will be effective when posted. Please check this Personnel Privacy Notice periodically for updates.

Types of Personal Information We Collect

“Personal Information” is information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, with you or your household. During the past 12 months, we may have collected the following categories of Personal Information about you:

  • Identifiers, such as your name, email address, physical address, phone number, photograph, government-issued IDs (such as your Social Security number, driver’s license number, visa and passport information), and work/residence permits.
  • Information within our records, such as your financial information, signatures, medical information, background check and drug screening information, health insurance information, insurance policy number, details of family members, and emergency contact details.
  • Characteristics of protected classifications under California or federal law, including your age, race/ethnicity, immigration/citizenship, national origin, marital status, gender, medical condition, veteran status, and disability information.
  • Biometric information, such as fingerprints for background check and credentialing purposes.
  • Internet or other electronic network activity information, such as browsing history, search history, and details of internet use held on or made through our systems, the contents of your communications, and general IT data (such as passwords, access rights, and usage data).
  • Non-precise geolocation data, such as your location derived from your device or your IP address when you use company systems.
  • Audio, electronic, visual, thermal, olfactory, or other similar information, such as photographs and information collected through recordings of meetings, member interactions, and events.
  • Professional or employment-related information, such as your title and compensation history, work schedule and status, work experience, education, licensure and other credentials, performance and other evaluation information, disciplinary information, resumes, human resources records, payroll, benefits and leave information, and information related to any employment-related legal issues or disputes that may arise.
  • Education information, such as education history, certifications, licenses, and other relevant skills.
  • Inferences drawn from any information we collect to create a profile about you regarding preferences, characteristics, etc.
  • Some of this information may be considered Sensitive Personal Information, including your government-issued IDs, financial account information, race/ethnic origin, the contents of your communications, biometric information, and health information.

Sources of Personal Information

We may collect Personal Information about you from the following sources:

  • Directly from you. We may collect Personal Information you provide to us directly, such as when you apply for a job, contact human resources, register for benefits, sign up for events, provide feedback, and through other interactions with us prior to, during, or after your employment or other professional working relationship with us.
  • Data collected automatically and through technologies. We may automatically collect information or inferences about you, such as through devices used in the context of your employment or other professional working relationship with us. This may include information about how you browse the internet, use company devices, and send and receive emails.
  • From third parties. We may collect Personal Information from third parties, such as companies that provide recruitment, staffing, benefits, payroll, travel, background check, drug screening, verification, or other services. We may also receive Personal Information from your colleagues or supervisors, for example, in the context of performance evaluations.


We may combine information that we receive from the various sources described in this Personnel Privacy Notice, including third party sources, and use or disclose it for the purposes identified below.

How We Use Personal Information

We may use Personal Information for a variety of purposes:

  • Employment-related activities, such as managing recruitment; processing job applications; evaluating candidates and hires; onboarding and offboarding; managing payroll, salary, payment, sick pay, and leaves of absences; tax and Social Security management; work eligibility monitoring; conducting background checks and drug tests; performance evaluation; scheduling, establishing emergency contact information; processing personnel work-related claims, and administering and processing benefits, including determining eligibility for you and your dependents and beneficiaries.
  • Legal, safety, or security reasons, such as in connection with proceedings or investigations, defending legal claims, maintaining records to comply with legal obligations, and receiving legal advice or approval.
  • Security and workplace monitoring, such as analyzing our electronic systems, networks and other equipment and property for security, quality and training purposes, including to protect against fraud and other illegal activity and ensure compliance with policies, and providing IT support to personnel.
  • Internal business and research purposes, such as enforcing our policies and procedures, performing analytics and applying algorithms to discover ways to improve our business, identify trends in our workforce, and enhance our service offerings.
  • In connection with a corporate transaction, such as if we acquire, or some or all of our assets are acquired by, another entity, including through a sale in connection with a merger or bankruptcy, or we engage in another form of corporate change.
  • Marketing or promotional purposes, such as externally promoting Vida and our brands, goods, or services.
  • Other purposes consistent with the context of the collection of your information, or as otherwise disclosed to you prior to the use of your information.


We only use Sensitive Personal Information for the following purposes, as permitted by applicable law: (i) performing services or provide goods the consumer would reasonably expect; (ii) detecting security incidents; (iii) resisting malicious, deceptive, or illegal actions directed at the business or consumers; (iv) ensuring the physical safety of individuals; (v) for short-term, transient use, including non-personalized advertising; (vi) performing or providing services on behalf of the business; (vii) verifying or maintaining the quality or safety of a service or device, or improving, upgrading, or enhancing the service or device; or (viii) collecting or processing Sensitive Personal Information where the collection or processing is not for the purpose of inferring characteristics about the consumer.

How We Disclose Your Personal Information

When we disclose your Personal Information, we do so to the following parties pursuant to the business purposes listed above.

  • Affiliates and subsidiaries, including parent entities; corporate affiliates, subsidiaries, and business units; and other companies that share common ownership with us.
  • Service providers or contractors that work on our behalf to provide products and services, such as IT and monitoring; payroll processing; onboarding; screening; learning and training; email and other communications systems; mailing and printing; maintenance and facilities management; data analytics and workforce assessment; financial services; co-employment, legal advice; and auditing.
  • Law enforcement, government agencies, and other recipients for legal, security, or safety purposes, such as when we share information to comply with law or legal requirements; to enforce or apply our policies; and to protect our, our customers’, or third parties’ safety, property, or rights.
  • Entities in connection with a corporate transaction, such as if we, or some or all our assets, are acquired by another entity, including through a sale in connection with bankruptcy, or other forms of corporate change.
  • Other third parties as reasonably necessary to complete transactions or provide services requested by you, such as insurance and benefits providers.
  • Other recipients with your consent.


We may disclose anonymized, de-identified, or aggregated information for any purpose permitted by law.

California Privacy Rights

If you are a California resident, you may have certain rights with respect to the Personal Information we maintain about you, including the following rights, subject to legal exceptions:

  • The right to know. You may have the right to request information about the categories of Personal Information we have collected about you, the categories of sources from which we collected the Personal Information, the purposes for collecting the Personal Information, and to whom we have disclosed Personal Information and why. You may also request the specific pieces of Personal Data we have collected about you.
  • The right to deletion. You may have the right to request that we delete Personal Information that we have collected from you subject to certain exceptions.
  • The right to correct. You may have the right to request that we correct inaccurate Personal Information that we maintain about you.
  • The right to opt out of sale or sharing. We do not sell Personal Information or share Personal Information for cross-context advertising purposes, including about individuals under 16 years old.
  • The right not to be retaliated against for the exercise of your privacy rights. We will not discriminate or retaliate against you if you exercise your rights as described above.

You can submit requests to exercise the rights above by calling us at 1 (855) 955-2439 or emailing us at compliance@vida.com to request to exercise the rights above. When you make a request to access or delete your information, we must first verify your identity before processing your request. We may use our current verification systems to verify your identify, or we may request additional information from you to verify against our records.

You can submit requests through an agent that you have authorized in writing to act on your behalf. Your agent will need your written and signed permission that they are authorized to act on your behalf. As explained above, your agent will also need to provide information verifying your and their identity. We may still need to contact you directly to verify your identity and the request, or if any other information may be needed.

Securing and Retaining Your Personal Information

Vida will take steps designed to protect your Personal Information against loss or theft, as well as from unauthorized access, disclosure, copying, use or modification, regardless of the format in which it is held and will use reasonable security safeguards to protect data.

We retain your Personal Information for as long as we need it to provide our products and services, operate our business, comply with our legal obligations, and fulfill the purposes outlined above unless we are required to do otherwise by applicable law. When we decide how long to keep your Personal Information, we keep in mind the nature and sensitivity of the information, the potential harm from unauthorized use, the reasons we collected the Personal Information, and our legal obligations.
